Landmark Home Health Care is a non-profit, Medicare certified agency dedicated to helping patients achieve the highest

service to each patient, we provide an accurate, continuous course of treatment that is closely monitored.

HOME HEALTH AIDES
Qualified and experienced home health aides can provide services that include grooming, bathing and other personal care
services under the supervision of a professional.
PHYSICAL THERAPY
Licensed physical therapists provide exercises, training, rehabilitation and establishment of a maintenance program.
SPEECH THERAPY
Experienced speech pathologists provide therapy for speech and hearing impaired patients. Swallowing disorder therapy,
voice exercises, language processing and memory strategies are provided.
OCCUPATIONAL THERAPY
Licensed occupational therapies and assistants provide Activies of Daily Living Training, homemaking skills, functional
ambulation, range of motion exercises and transfer training.
MEDICAL SOCIAL SERVICES
Social workers evaluate the patient's personal and financial needs and link patients with sources of assistance. They also
provide counseling to patients and their family caregivers.
REGISTERED DIETICIAN
Available for consultation regarding special diets and nutritional care of patients.
